The terminology linked numbering scheme is not used in the North American Numbering Plan (NANP). A local call in this scheme, standardized in 1948 for the introduction of direct-dial long- distance calling, was originally dialled as a fixed-length seven-digit number and did not require an area code. In some cases, a local call to a number just across an area code boundary could be seven digits if an exchange code protection scheme prevented the same prefixes being assigned to local numbers in the other area code. A long-distance trunk call within the same area code used to be dialable as 1 followed by seven digits, without the area code.

Early crossbar exchanges were divided into an originating side and a terminating side, while the later and prominent Canadian and US SP1 switch and 5XB switch were not. When a user picked up the telephone handset, the resulting line loop operating the user's line relay caused the exchange to connect the user's telephone to an originating sender, which returned the user a dial tone. The sender then recorded the dialed digits and passed them to the originating marker, which selected an outgoing trunk and operated the various crossbar switch stages to connect the calling user to it. The originating marker then passed the trunk call completion requirements (type of pulsing, resistance of the trunk, etc.) and the called party's details to the sender and released.

A long-distance call is a "trunk call" in British English, but is a "toll call" in American English, though neither term is well known among younger Americans. The distinction is a result of historical differences in the way local service was billed; the Bell System traditionally flat-rated local calls in all but a few markets, subsidising local service by charging higher rates, or tolls, for intercity calls, allowing local calls to appear to be free. British Telecom (and the British Post Office before it) charged for all calls, local and long distance, so labelling one class of call as "toll" would have been meaningless. Similarly, a toll-free number in America is a freephone number in the UK. The term "freefone" is a BT trademark.

A different intercept message is used when the caller has dialed a number the first three digits of which have not yet been assigned within that area code. This message, too, formerly varied by locality; the following was used in New York City in the 1960s: The standard version of this message in use today is the following: Most of these messages often include the phrase "Your call cannot be completed as dialed." Sometimes a message would say to first dial a 1 or a 0 (the toll prefix for a trunk call) plus the area code of the called number. A message may also be played when 0 or 1 followed by an area code is prepended unnecessarily on landline phone calls to local destinations.
