Appendix cancer are very rare cancers of the vermiform appendix. Gastrointestinal stromal tumors are rare tumors with malignant potential. Primary lymphomas can occur in the appendix. Breast cancer, colon cancer, and tumors of the female genital tract may metastasize to the appendix.

In very rare cases, the appendix may not be present at all (laparotomies for suspected appendicitis have given a frequency of 1 in 100,000). Sometimes there is a semi-circular fold of mucous membrane at the opening of the appendix. This valve of the vermiform appendix is also called Gerlach's valve.

De Garengeot's hernia is a rare subtype of an incarcerated femoral hernia. This eponym may be used to describe the incarceration of the vermiform appendix within a femoral hernia. Akopian and Alexander, named this hernia after the 18th century Parisian surgeon Rene Jacques Croissant de Garengeot. He is quoted in the surgical literature as the first to describe this situation in 1731.

These body parts can be classed as additional to the required functioning of the body. In human anatomy the vermiform appendix is sometimes classed as a vestigial remnant. The appendicular artery, also known as appendiceal artery, commonly arises from the terminal branch of the ileocolic artery, or less commonly from the posterior cecal artery or an ileal artery. It descends behind the termination of the ileum and enters the mesoappendix of the vermiform appendix. It runs near the free margin of the mesoappendix and ends in branches which supply the appendix. He was first Principal Surgeon to the Westminster Hospital, and founder and first Principal Surgeon to St George's Hospital. On 5 April 1716 he was elected a Fellow of the Royal Society under the name Claude Amyand. In 1722, he inoculated three of the children of the Prince and Princess of Wales against smallpox, following the introduction of the process into the country from Turkey by Lady Mary Wortley Montagu. At St George's Hospital, on 6 December 1735, he performed the first recorded successful appendicectomy (which is the surgical removal of the vermiform appendix).

In humans the vermiform appendix is a vestigial structure; it has lost much of its ancestral function. Vestigiality is the retention during the process of evolution of genetically determined structures or attributes that have lost some or all of the ancestral function in a given species. Assessment of the vestigiality must generally rely on comparison with homologous features in related species. The emergence of vestigiality occurs by normal evolutionary processes, typically by loss of function of a feature that is no longer subject to positive selection pressures when it loses its value in a changing environment.

According to the text, the stomach is spherical; the stomach wall has an internal lining, which is "the seat of sensation", and an external fleshy coat that is involved in digestion. In order to access the spleen, which was thought to secrete black bile into the stomach through imaginary canals, the dissector was required to remove the "false ribs". The liver is said to have five lobes, the gallbladder is described as the seat of yellow bile, and the cecum is described with no mention of the vermiform appendix.

The issue is controversial and not without dispute; nonetheless, vestigial organs are common evolutionary knowledge. In addition, the term vestigiality is useful in referring to many genetically determined features, either morphological, behavioral, or physiological; in any such context, however, it need not follow that a vestigial feature must be completely useless. A classic example at the level of gross anatomy is the human vermiform appendix—though vestigial in the sense of retaining no significant digestive function, the appendix still has immunological roles and is useful in maintaining gut flora. Similar concepts apply at the molecular level—some nucleic acid sequences in eukaryotic genomes have no known biological function; some of them may be "junk DNA", but it is a difficult matter to demonstrate that a particular sequence in a particular region of a given genome is truly nonfunctional.
