COH is carried out either in an agonist protocol, using gonadotrophin-releasing hormone (GnRH) analogues for pituitary desensitisation, combined with human menopausal gonadotrophins (hMG) or recombinant follicle stimulating hormone (FSH), or an antagonist protocol using recombinant FSH combined with a GnRH antagonist according to clinical assessment of the patient's profile (age, body mass index (BMI), endocrine parameters). hCG is administered when at least three follicles of more than 17 mm mean diameter are seen at transvaginal ultrasound scan.

At early presentation of pregnancy at around 6 weeks, early dating ultrasound scan may be offered to help confirm the gestational age of the embryo and check for a single or twin pregnancy, but such a scan is unable detect common abnormalities. Details of prenatal screening and testing options may be provided. Around weeks 11–13, nuchal translucency scan (NT) may be offered which can be combined with blood tests for PAPP-A and beta-hCG, two serum markers that correlate with chromosomal abnormalities, in what is called the First Trimester Combined Test. The results of the blood test are then combined with the NT ultrasound measurements, maternal age, and gestational age of the fetus to yield a risk score for Down Syndrome, Trisomy 18, and Trisomy 13.

Whitfield had more success in gaining acceptance for obstetric ultrasound than Donald himself, who was sometimes viewed as an obsessive eccentric. As he recalled, "an honorary attachment with Professor Donald in Glasgow in 1964 had convinced me that obstetric ultrasound did have a future, but later in that year in America I was told it was just a dream of a mad, red-headed Scotsman, so I should forget it! I was told confidentially, nothing, no good will come of it, but of course that was wrong". By the end of the 1980s, 87% of pregnant women in Scotland would undergo at least one ultrasound scan during the course of their pregnancy, a task increasingly undertaken by midwives (as opposed to doctors or radiologists), an innovation first introduced at QMH.
