Kohut departed from the classical Freudian view, which suggested that some patients could not be analyzed given that they lacked the ability to develop transferences. He postulated that narcissistic patients are capable of presenting transferences but these are somewhat different from those of other patients, such as the neurotics. He distinguished three types, namely the idealizing, the mirror, or the twinship transference. His debate with Kernberg concerns mostly the idealizing transference, which, according to Kohut, relates to a fixation at an archaic level of normal development.

An extension of Freud's theory of narcissism came when Heinz Kohut presented the so-called "self-object transferences" of idealization and mirroring. To Kohut, idealization in childhood is a healthy mechanism. If the parents fail to provide appropriate opportunities for idealization (healthy narcissism) and mirroring (how to cope with reality), the child does not develop beyond a developmental stage in which he sees himself as grandiose but in which he also remains dependent on others to provide his self-esteem.Corbett, L. The resistance comes from the patient's mis-perception of the therapist as a hostile opponent. Why would any patient feel comfortable and reveal material to a therapist who they mis-perceive in this way? Thus, transferences provoke more resistance, which is added to the two prior sources. Those patients with the most have richly suffused unconscious structures cannot see the therapist for who he/she is, and will display the most resistance, as the inner vision will override the weakened perceptions of the central ego.

Kohut had a significant impact on the field by extending Freud's theory of narcissism and introducing what he called the 'self-object transferences' of mirroring and idealization. In other words, children need to idealize and emotionally "sink into" and identify with the idealized competence of admired figures such as parents or older siblings. They also need to have their self-worth mirrored by these people. These experiences allow them to thereby learn the self-soothing and other skills that are necessary for the development of a healthy sense of self.

Transference neurosis is a term that Sigmund Freud introduced in 1914 to describe a new form of the analysand's infantile neurosis that develops during the psychoanalytic process.(Etchegoyen, 1991) Based on Dora's case history, Freud suggested that during therapy the creation of new symptoms stops, but new versions of the patient's fantasies and impulses are generated. He called these newer versions "transferences" and characterized them as the substitution of the analyst for a person from the patient's past. According to Freud's description: "a whole series of psychological experiences are revived not as belonging to the past, but as applying to the person of the analyst at the present moment".

With favourable development, grandiosity will develop into ambitions of an adult person, and the idealizing attitude will develop into our deepest ideals, against which we measure ourselves and from which we expect guidance and directions for our actions. Childhood trauma interfere with these developments, and these configurations are repressed and their psychological energies are not available to the individual, resulting in a low self-esteem. It is precisely these damaged structures of the self that are activated in selfobject transferences, and they will then be drawn into the processes of transmuting internalization, and in time they will result in a healthy self-esteem. In their textbooks for actors, both Stanislavski and Hagen adhere to a mode of theatrical performance that starts with the subjective experience of the actor, who takes action under the circumstances of the character, and trusts that a form will follow. They deem it more useful for the actor to focus exclusively on the fictional, subjective reality of the character (via the actor's "emotional memory" or "transferences" from his own life), without concerning himself with the external realities of the theatre. When there are disruptions in the transference, the patients feel unreal and numb, and they have hypochondriac fears and their energies are gone. The central problem of these patients lie in their self-esteem, which is low and unreliable, and they are vulnerable and easily irritated.Strozier 2001, p.196. The narcissistic transferences are new versions of childhood human relations gone awry. With the grandiose self, the attitude is “I am perfect”, whereas with the idealized parent imago it is “you are perfect, but I am part of you”. These attitudes can be discerned in children and in psychoanalytic patients when they have attained the level of the cohesive self.
