On August 4, 2007, at the Fête des Loges in Saint-Germain-en-Laye, France, a gondola broke loose and crashed to the ground. Two people (a father and son) in the gondola were killed and two seriously injured. Two other people were stuck 40 meters in the air for over two hours. Same problem at the funfair of Rennes, France, in the night of the new year, a technical problem had blocked the ride but not the safety harness system, 8 people who were in the basket located at the top of the ride at 55 were rescued by a helicopter.

On June 17, 1987, a 19-year-old woman from Chester, Pennsylvania fell to her death as a result of not being properly secured by the over-the-shoulder harness. She was pronounced dead at a nearby hospital with Basilar skull fracture suffered from the fall. Early reports indicated that the woman tried to board late after the safety harness was locked and the attendant couldn't stop the train. An investigation found the ride itself to be operating properly, but that an error was made by the ride operator, who started the ride without ensuring all passengers were secured.

Jules Léotard, inventor of the flying trapeze In the early years of young Mr. Leotard's performances, the flying trapeze did not have the safety net as is typically seen today. He would perform over a series of mattresses on a raised runway to give the audience a better view of his tricks, or "passes". Most modern flyers start out wearing a safety harness, while a trainer on the ground controls the lines and would pull them if the flyer is in a dangerous situation. In 1986, a man died during rehearsals for a bungee jumping stunt on a BBC television programme, because the cord sprang loose from a carabiner clip. Three days before the race, several drivers, Formula E Teams Association representative and Andretti team principal Roger Griffiths and FIA officials discussed in a meeting procedures of improving the safety of car switches during the mandatory pit stops. It was called after teams were issued clarifications and remainders of pit stop procedures and possibly in response to an injury sustained by a Techeetah mechanic by driver André Lotterer in the preceding Mexico City ePrix. These clarifications included keeping the steering wheel installed in the driver's first car until it stopped and the FIA wanted all teams not to fully install the safety harness until the driver entered their second vehicle. In endurance racing, driver changes are mandatory; the shortest endurance races are scheduled for four hours, one hour longer than the longest nonstop time permitted behind the wheel. Graydon was shorter than Moore at 5 ft 8in but it was necessary to maintain the height difference with the stuntman doubling for 7 ft 2in Richard Kiel who played Jaws. In the scene he had to fall and then hold on to the roof of the cable car with his hands and did not have his safety harness on.

Porsche 991 GT3 RS Porsche launched the Renn Sport version of the 911 GT3 at the Geneva Motor Show in 2015. Compared to the 911 GT3, the front fenders are now equipped with louvers above the wheels and the rear fenders now include Turbo-like intakes, rather than an intake below the rear wing. As top speeds increased, more attention was paid to driver safety, much like with driver safety programs being instituted in most motor racing series today, such as NASCAR, Formula One, INDYCAR, Grand Prix motorcycle racing, and others. Almost immediately, passengers on previous flights pointed out the "complicated system of straps, carabiners and an emergency blade for cutting [the proprietary eight-point Safety Harness System] off in case of trouble" and inadequate training as probable causes for the passengers' deaths in a March 12 New York Times article about the crash.
