Roger Ebert of the Chicago Sun-Times awarded the film 3½ stars of four, calling it "a real movie. That means carefully written dialogue and carefully placed supporting performances — and it's about something. It could have easily been a formula film...but George Simmons learns and changes during his ordeal, and we empathize." It is the highest rating Ebert ever gave an Adam Sandler film, tied with his review for Punch-Drunk Love.
Jon Brion provided the score and seven original songs for the film. His unique methods for writing previous film scores (Punch-Drunk Love, Eternal Sunshine of the Spotless Mind) involved very close collaboration with the director. Through this process, David O. Russell was able to sit in the same room with Brion and watch an early cut of the film. Russell described what he wanted to portray and Brion would compose music to Russell's descriptions.

He was known for roles such as the tough, experienced mentor of Elvis Presley's character in Jailhouse Rock and Maxi the punch-drunk boxer in Designing Woman (1957) opposite Gregory Peck. The New York City-born Shaughnessy was a Golden Gloves boxer. Shaughnessy started out as a singer before becoming a comedian.

Chronic traumatic encephalopathy, or "CTE", is an example of the cumulative damage that can occur as the result of multiple concussions or less severe blows to the head. The condition was previously referred to as "dementia pugilistica", or "punch drunk" syndrome, as it was first noted in boxers. The disease can lead to cognitive and physical handicaps such as parkinsonism, speech and memory problems, slowed mental processing, tremor, depression, and inappropriate behavior. Jack Palance portrays Harlan "Mountain" McClintock, a once-promising but now washed-up boxer who faces the end of his career after he is savagely defeated by a younger boxer. Keenan Wynn portrays McClintock's manager Maish; Keenan's father Ed plays McClintock's cut man, Army. McClintock is suffering from Dementia pugilistica or "punch drunk syndrome"—brain damage caused by his career. A fight doctor refuses to certify McClintock for further boxing, saying that another rough match could blind or even kill him.

Dr. Bennet Omalu, a forensic neuropathologist, examined tissue from Webster and eight other NFL players and determined they all showed the kind of brain damage previously seen in people with Alzheimer's disease or dementia, as well as in some retired boxers. Webster's brain resembled those of boxers with "dementia pugilistica", also known as "punch-drunk syndrome".

Brion's five-year relationship with comedian Mary Lynn Rajskub had abruptly ended during the shooting of the Paul Thomas Anderson film Punch-Drunk Love (2002), which Brion was scoring. The new building of the Newark City Hospital was named the Martland Medical Center and is now part of Rutgers University Harrison Stanford Martland (September 10, 1883 – May 1, 1954) was an American pathologist who coined the term punch drunk. Martland was born in Newark, New Jersey. In 1901 he received a BS degree from Western Maryland College and in 1905 he received his MD degree from Columbia University College of Physicians and Surgeons. Dr. Martland accepted the appointment of the first full-time paid pathologist at the Newark City Hospital in January 1909.

In early 1931 Fontaine travelled from Canada to the United Kingdom by cattle boat to further his boxing career. Over the next three years Fontaine faced many of Britain's most successful middleweight boxers, including Billy Bird, Jack Casey, Gipsy Daniels, Tommy Farr, Jack Hyams and Harry Mason. From his arrival in the U.K., Fontaine's fight record was good with 23 wins, 4 losses and 3 draws, but from November 1933 his form took a terrible reversal with 4 wins, 16 losses and 2 draws. This string of poor results would later be used as Fontaine's defence during his murder trial, with his defence counsel stating he was "punch drunk" and therefore of diminished responsibility.

Not much is known about "Martini"; it is regarded as just another song that didn't make the album. "Pantomime" and "Monuments and Melodies" (and the Japanese bonus track "Follow") appear on Incubus's Alive at Red Rocks DVD from 2004 as tracks on the bonus CD, though the "Pantomime" in this compilation is an entirely different recording featuring the whole band for the entire song, whereas it is revealed in the concert section of the Alive at Red Rocks DVD and the studio version featured on the bonus CD that the first half of the song is solely Brandon playing guitar and singing.
