By now, the story of the opioid epidemic is well-known: In the 1990s, drug manufacturers marketed opioids aggressively — particularly Purdue Pharma with the newly released OxyContin, which the company suggested was safer and less prone to misuse than other opioids.
"These drug manufacturers led prescribers to believe that opioids were not addictive, that addiction was an easy thing to overcome, or that addiction could actually be treated by taking even more opioids," Ohio Attorney General Mike DeWine said in a statement.
And some insurers are now actually paying doctors not to prescribe opioids: Blue Cross Blue Shield of Michigan increases the amount it will pay for specific surgeries by 35 percent if surgeons only prescribe a minimal amount of opioids afterwards.
A little under a third were caused by prescription opioids, a quarter by heroin, and about a third by synthetic opioids other than methadone — most of which were likely caused by fentanyl, an extremely potent synthetic opioid mostly manufactured in China.
The authors say the study is the first of its kind to make quantitative associations between how much pharmaceutical companies spent marketing opioids and their influence on the number of prescriptions and the number of fatal overdoses from prescription opioids.
"When I talk to parents, there is a certain amount of caution about opioids and parenting, but part of the reason for seeking out opioids is to provide relief to their painful connection to the world around them," she said.
Two years ago, the Centers for Disease Control and Prevention released the first national guidelines for prescribing opioids, recommending that doctors try pain relievers like ibuprofen first and that they give most patients only a few days' supply of opioids.

But the potential for addiction to prescribed opioids is real: 20 percent of patients who receive an initial 10-day prescription for opioids will still be using the drugs after a year, according to a recent analysis by the CDC.
"So at the very time you're educating them about the appropriate prescribing of opioids, you're also educating them about how to spot signs of abuse and treat it If they do have a patient who becomes addicted to opioids," Gottlieb said.
Dr. Adams said making naloxone more available in communities across the country is critical to reducing overdose deaths among people prescribed high doses of opioids for pain, as well as those who abuse painkillers or illicit opioids like fentanyl and heroin.
This cultural shift led to exponential growth in the quantity of opioids prescribed for chronic non-cancer pain, an increase in the average length of time opioids were prescribed and higher dosages used by patients for a long period of time.
"The fact that these groups registered their opposition while receiving funding from the opioids industry raises the appearance -- at the very least -- of a direct link between corporate donations and the advancement of opioids-friendly messaging," the new report states.
For example, the study shows that nearly 5% of middle-age marijuana users had alcohol use problems, 9% depended on nicotine, and 3.5% misused opioids; among older adults, 20023% had alcohol use problems, 3.5% depended on nicotine, and 1.2% misused opioids.
But while medical opioid use has fallen by nearly one-third since peaking around 2011 — and deaths associated with prescription opioids have stabilized — overall opioid overdose fatalities have recently hit a high as more potent, illegally manufactured opioids hit the streets.
