Specifically, weakening occurs in the paravertebral muscles of patients. These paravertebral muscles have a great influence over the walking stance and gait of a patient, so fatty infiltration and degradation of these muscle lead to the characteristics that easily define BSS, such as the anterior flexion of the back combined with an ability to keep upright with any kind of support (e.g., holding onto a table). Secondary BSS can have a multitude of causes, making it hard to pinpoint to a specific muscular disorder.

A top- heavy bike can require more effort to keep upright, when stopped in traffic for example, than a bike which is just as tall but with a lower center of mass. This is an example of a vertical second-class lever. A small force at the end of the lever, the seat or handlebars at the top of the bike, more easily moves a large mass if the mass is closer to the fulcrum, where the tires touch the ground. This is why touring cyclists are advised to carry loads low on a bike, and panniers hang down on either side of front and rear racks.

Note: Graham Anderson in his 1999 book Fast Light Boats, a Century of Kiwi Innovation argues that planing centreboard sailing boats were introduced into New Zealand in the early 20th century – well before Uffa Fox popularised the concept. Another advance in dinghy sailing was introduced in the 1930s, when the technique of trapezing was introduced. This involves using the crew to provide more leverage (than possible by hiking out) to keep the sails vertical and the boat balanced; doing this can prevent the boat from capsizing, by hanging outside the boat on a harness and rope attached to the 'hounds' or upper mast. As a result, the boat is easier to keep upright, and the sails can deliver maximum power most of the time.

In general, however, lower-extremity spasticity in spastic diplegia is rarely so great as to totally prevent ambulation—most people with the condition can walk, and can do so with at least a basic amount of overall stability. Regardless, from case to case, steeply varying degrees of imbalance, potential tripping over uneven terrain while walking, or needing to hold on to various surfaces or walls in certain circumstances to keep upright, are typically ever-present potential issues and are much more common occurrences amongst those with spastic diplegia than among those with a normal or near-normal gait pattern. Among some of the people with spastic diplegia who choose to be ambulatory on either an exclusive or predominant basis, one of the seemingly common lifestyle choices is for the person to ambulate within his or her home without an assistive device, and then to use the assistive device, if any, once outdoors. Others may use no assistive device in any indoor situation at all, while always using one when outdoors.
