In the country's early years, most drug use by the settlers was of alcohol or tobacco. The 19th century saw opium usage in the US become much more common and popular. Morphine was isolated in the early 19th century, and came to be prescribed commonly by doctors, both as a painkiller and as an intended cure for opium addiction. At the time, the prevailing medical opinion was that the addiction process occurred in the stomach, and thus it was hypothesized that patients would not become addicted to morphine if it was injected into them via a hypodermic needle, and it was further hypothesized that this might potentially be able to cure opium addiction.

Scott and colleagues therefore developed EMG-guided injection, a system that uses a hypodermic needle that records the electrical activity of the muscle (the electromyogram or EMG) at its tip. The needle is introduced under local anesthesia, and as the awake patient looks in different directions, the pattern of muscle activity, played through a speaker, indicates when the belly of the target muscle has been entered, and the drug is then injected. Modern techniques of ultrasound examination now allow the extent and localisation of an attack within timbers to be determined with great accuracy, and, for historic properties where damage to ornate plasterwork must be avoided, can be followed by micro-drilling and highly- targeted injection of insecticide via hypodermic needle.

On March 13, 2019, June Shannon and her boyfriend, Geno Doak, were both arrested at a gas station in Alabama, after police were called to the scene following reports of a public dispute. Both June and Geno were arrested and charged with possession of a controlled substance, reportedly crack cocaine, and possession of drug paraphernalia, with a glass pipe found on June and a hypodermic needle found on Geno. Geno also faced an additional charge of domestic violence and harassment in relation to the public altercation with June. The two were indicted on felony charges related to drug possession and Doak with third-degree domestic violence on September 13, 2019, though later reports do not mention a needle being found in Geno's possession, only the pipe, which June reportedly told police belonged to her.

Cotton fever may bear resemblance to other bodily infections such as the flu; a culture would be required to determine whether a particular patient has been infected by the specific bacteria (Pantoea agglomerans) that causes true cotton fever. Confusion continues to exist as outdated or inaccurate definitions of the condition, including its causes, remain widespread, especially among intravenous drug users, who are likely to self-diagnose based on local common knowledge. Although it cannot be ruled out, cotton fever could possibly result from direct introduction of a small cotton fiber or other particulate matter into the bloodstream which may have attached itself to the tip of a used or blunted hypodermic needle. Another possibility is seen by the increased infection rate due to cotton particles introducing bacteria directly into the bloodstream.

The office's primary treatment, called intracavernous pharmacotherapy (ICP), consists of administering an injection of a vasodilator drug by inserting a hypodermic needle directly into the penis, which increases blood flow, and causes an erection in minutes in many patients. ICP treatment has been used to help create and maintain erections in patients who do not respond to more mainstream medications or are prohibited from taking these drugs because of a heart condition, diabetes or blood pressure since ICP treatments are localized and do not typically interact with other systemic medications. Edgar Hoover began a smear campaign to the press, to subordinate FBI agents and to select politicians, claiming the cut marks from the car's shattered window were "puncture marks in her arm indicating recent use of a hypodermic needle; she was sitting very, very close to that negro in the car; that it has the appearance of a necking party." While attempting to obscure the fact that an FBI informant was in the car, and to ensure that the FBI was not held responsible for permitting their informant to participate in violent acts, without FBI surveillance or backup, the FBI was concerned that they might be held accountable for their informant's (Rowe) role in the death. Rowe had been an informant for the FBI since 1960. The FBI was aware that Rowe had participated in acts of violence during Ku Klux Klan activities.

Lee Kuan Yew of Singapore, Hindu nationalists in India, and the Chinese authorities have all pressed for restrictions on Western media in their respective nations, viewing it as a possible threat to Asian values. Other scholars, such as Fred Fejes, Daniël Biltereyst and Hamid Naficy, have criticized the "media imperialism" theory, arguing that it unjustifiably relies on a "hypodermic needle model" of media effects, overstates the influences of media on the audience's behavior and views, and romanticizes "national culture" unduly.

A needlestick injury is the penetration of the skin by a hypodermic needle or other sharp object that has been in contact with blood, tissue or other body fluids before the exposure. Even though the acute physiological effects of a needlestick injury are generally negligible, these injuries can lead to transmission of blood-borne diseases, placing those exposed at increased risk of infection from disease causing pathogens, such as the hepatitis B virus (HBV), hepatitis C virus (HCV), and human immunodeficiency virus (HIV). Among healthcare workers and laboratory personnel worldwide, more than 25 blood- borne virus infections have been reported to have been caused by needlestick injuries. In addition to needlestick injuries, transmission of these viruses can also occur as a result of contamination of the mucous membranes, such as those of the eyes, with blood or body fluids, but needlestick injuries make up more than 80% of all percutaneous exposure incidents in the United States.

Gabrielle Lavallée herself underwent harsh treatment at the Ontario commune during the late 1980s, suffering welding torch burns to her genitals, a hypodermic needle breaking off in her back, and even eight of her teeth being forcibly removed. Lavallée attempted to escape from the commune after Thériault cut off parts of her breast and smashed her head in with the blunt side of an axe, but upon her return he removed one of her fingers with wire cutters, pinned her hand to a wooden table with a hunting knife, and then used a chainsaw to amputate her entire arm. In 1989, Thériault was arrested for assault after Gabrielle Lavallée had fled the commune again and contacted authorities, effectively dissolving the Ant Hill Kids. Provincial authorities had long-held suspicions against Thériault's cult due to the particularly primitive living conditions of its membership, but because the commune was officially registered as a church, officials were legally unable to investigate the adults, and could not do much except ensure the welfare of the children.
