Upon their arrival in a seemingly abandoned small town, however, they are attacked by the demon Meg and a pack of hellhounds.

At the end of the third season of Supernatural, main character Dean Winchester is in Hell after being killed by the hellhounds of the demonic antagonist Lilith. In the season four premiere "Lazarus Rising", the angel Castiel is introduced as the one who brought Dean back from Hell and resurrected him. Because merely perceiving his actual form typically results in blindness, he takes a human host – James "Jimmy" Novak, a "devout man" who prayed for it – to communicate with Dean, and tells him that he has been brought back because God has work for him. Lilith is breaking the 66 seals in order to free Lucifer, and Dean must stop her.

As they fight, a disoriented Maria stumbles upon them; she pretends to be her own ghost to frighten and shock them out of the duel. Afterward her Nurse takes her in, under the disguise of a Welsh cousin; the Lady soon penetrates the guise, but agrees to maintain it till Maria's reputation can be restored. Lurcher and the boy, under various disguises, fool and rob the Justice, and then waylay and drug him; when the Justice wakes, he is confronted with "furies" and "hellhounds" ready to drag him to Hell for his sins. His soul is saved, however, when an angel (the boy in costume) intervenes; Justice Algrip repents his past misdeeds and promises to make amends.

Duke, portrayed by Aidan Kahn, is a rogue Crossroads Demon appearing in season 11 who sets out for his own benefit following the release of the Darkness and Lucifer. In "Beyond the Mat," Duke forces wrestler Gunner Lawless to help him murder people and steal their souls so that Duke can create a "nest egg" for himself with the release of the Darkness and Lucifer ruling Hell again. Having made a crossroads deal with Gunner ten years before, Duke offers to keep the hellhounds at bay if Gunner works for him, a deal Gunnar reluctantly takes. The Winchesters come to the funeral of one of Duke's victims, their father's favorite wrestler and stumble across the crime scene of his newest target, quickly putting together that they are dealing with a demon stealing souls.

However, Dean and the Winchesters' ally Bobby Singer realize that Sam is possessed, and despite Meg's attempt to seal herself in Sam using a binding link, she is forced to flee Sam's body once she becomes vulnerable to exorcism after Dean and Bobby burn the mark off. After the demons' god Lucifer is freed from Hell, Meg (Rachel Miner) resurfaces in the fifth season premiere "Sympathy for the Devil" to try to kill Dean (as he is believed to be the one who will kill Lucifer) before he can find a weapon said to be capable of killing Lucifer. Her attack leaves Bobby paralyzed, but she is forced to flee when Dean tries to kill her. She returns in "Abandon All Hope..." and sends a pack of hellhounds after Sam, Dean, and their friends Jo and Ellen Harvelle when they arrive to kill Lucifer; the confrontation ends in Jo and Ellen's deaths.

The Enchantress then helps Ragman defeat an assassin sent to kill him, in the course of which they are attacked by the Wild Hunt of legend and temporarily transformed into mystical hellhounds. Shortly after, her spells are all that save Jim Rook, Nightmaster, after he is stabbed by his own sword fighting Etrigan the Demon; staying at his side for days without sleep and keeping him alive by trading a day of her life for a day of his until he heals himself by the power of his sword. After Shadowpact are seconded to Checkmate to infiltrate Kobra's organization, she then helps foil Doctor Gotham's plan to destroy Chicago by entering his inter-dimensional cloak and destroying most of the thousands of magical artifacts contained within. She takes an apprentice, Laura Fell, the Warlock's Daughter before they both unwittingly almost allow a race of mindless creatures "The Unbound" to reach the Earth while creating a portal to the Land of Nightshades, where Nightmaster, Nightshade, and Ragman are trapped.
