Beecher learns of Schillinger's actions and, with the help of Whittlesey and McManus, gets Schillinger's parole hearing canceled and his sentence extended by ten years for plotting to kill him. Schillinger is moved out of Emerald City permanently. In gen pop, Schillinger is then brutally attacked by black inmates. He and a fellow Aryan, Mark Mack, decide to randomly kill a non-white inmate to win back respect for the Brotherhood, targeting Jewish inmate Alexander Vogel.

Jessica is reluctant and stalls then James refuses to be a rape Jessica and she is taken away. Jason privately meets with Jessica and wants to take her away but she refuses, accepting her fate. She does ask to meet James, whom she would like to thank for being a gentleman, and Jason arranges this meeting, where James and Jessica make love. Sarah later throws Jason into Gen Pop now that the governor cannot protect him, but not before having his arm bled, making him vulnerable to the women vampires.

In gen pop, Nicky arrives at C-Block and drops off the supplies needed to inseminate Blanca, she later shares a conversation with Red. Nicky helps Flores attempt to get pregnant. However, since Flores had to buy drugs off of C-block and gave them to D-block in order to get Diablo's semen snuck in, they get beat up during the insemination process in the library by Brock, Charlene Teng, Akers, Creech and Shruti Chambal. They end up in the hospital ward, but Flores is released early to make room for Barbara, who OD'd on bath salts.

Glynn finds most of the demands to be reasonable, but Devlin urges Glynn to have the S.O.R.T. team take back Em City force. McManus opposes this and goes into Em City with food for the prisoners in order to see how the hostages are doing. When McManus does not return, Devlin has the S.O.R.T. team go in with machine guns and a violent battle ensues. As a result, 6 inmates and 2 correctional officers die and the rest of the prisoners are sent back to Gen Pop (With the exception of the riot leaders who were taken to Solitary).

When he has to pass another test by killing Zee, Alex starts to remember his name so he attempts to escape for the third time along with Zee and Simon, this time through the elevator that connects the prison with the main gate. After a while, he starts to feel weak due to the lack of the nectar; without it he can either become a boy again or completely lose his mind and become a merciless killer, worse than a berserker, but Simon takes care of it. In the prison's 'gen pop' they and the other prisoners successfully defeat their enemies sent - first the blacksuits, later two berserkers - and reach the main gate. They are finally free.

At the end of the fall finale, Bellick reluctantly accepts a deal of 25 years to avoid the death penalty and is sent to Fox River to begin serving his sentence. His last scene of the episode shows him in his cell in Gen Pop. The character spends the next two episodes behind bars. As an inmate of Fox River, Bellick attempts to stay out of trouble but is bullied by the other inmates in the episode "John Doe", an inmate called Banks forces Bellick to bring him his dessert after meals, Bellick does this but Banks demands more from Bellick who subsequently attacks him and his entourage with a makeshift blackjack, he immediately is feared and respected by the other inmates because it appears that Banks is high up in the prison food chain. It is revealed in the episode ”The Message” that Banks and his crew have got their revenge on Bellick, because he has been brutally beaten overnight, he is also warned that there are many more beat downs to come.
