The cockpit reference manual contained no guidance for the crew on how to respond to the engine starter warnings. The crew proceeded to take off without realizing that the No. 2 engine starter was still engaged. Around 3 minutes and 45 seconds after taking off from Irkutsk, at an altitude of 4,000 ft, the No. 2 engine suddenly failed. The starter had continued to operate at high revolutions (over 40,000 rpm) with open air bleed valves from the engines, resulting in a critical failure of the starter.
