The ascent of Creag Nay presented issues of curling tracks, poor terrain and steep ascents. It was decided a rack and pinion system would be most effective. Flexible cogged belt was stuck on a wooden baton which was glued to the centre of a track. This engaged with a cog wheel fitted on the axle of the Silver Lady.

From July 2, 1929, the new electrically powered vehicles ran every 15 minutes. During and after World War II the line became very run down, and closure became a possibility. However this was averted, and in 1973 the line was completely rebuilt to celebrate the 100th anniversary of the merger of Buda, Pest and Óbuda into Budapest. The track was renewed using the Strub cog-wheel system, the line voltage was increased to 1500V and new vehicles, built by SGP and BBC, introduced.

The Aigle–Leysin railway (, AL) was the earliest of the narrow gauge line in the Chablais area of south west Switzerland. The locomotive comprised a boiler with a single return flue mounted on a four-wheel frame. At one end, a single cylinder with very long stroke was mounted partly in the boiler, and a piston rod crosshead ran out along a slidebar, an arrangement that looked like a giant trombone. As there was only one cylinder, this was coupled to a large flywheel mounted on one side. The rotational inertia of the flywheel would even out the movement that was transmitted to a central cog-wheel that was, in turn connected to the driving wheels.

The Salamanca locomotive The first commercially successful steam locomotive was Matthew Murray's rack locomotive Salamanca built for the Middleton Railway in Leeds in 1812. This twin-cylinder locomotive was not heavy enough to break the edge-rails track and solved the problem of adhesion by a cog-wheel using teeth cast on the side of one of the rails. Thus it was also the first rack railway. The first wave of construction conducted throughout early 1960s reignited conversations among students and citizens about the future of UAC with respect to the main campus in Edmonton. On 8 November 1963, a massive student protest dubbed "the Great Autonomy Demonstration of '63" occurred during a UAC Board of Governors' meeting, where protesters demanded autonomy for the budding UAC. The demonstration featured a headless mannequin with the words "Give UAC her head" and an engineering cog wheel with the words "Autonomy 196?." Clydebank District Council was granted new arms on 3 September 1975, consisting of a red saltire on a white field for the ancient province of Lennox and for the town's more recent historic links to Ireland which previously used the same flag. The cog-wheel symbolised all the local industries and the demi-figure of Saint Patrick referred to Old Kilpatrick, a burgh of barony from 1672, and where the saint is reputed to have been born.

Blenkinsop rack and pinion with teeth on outer side of one rail only John Blenkinsop thought that the friction would be too low from metal wheels on metal rails, so he built his steam locomotives for the Middleton Railway in 1812 with a 20-tooth, diameter cog wheel (pinion) on the left side that engaged in rack teeth (two teeth per foot) on the outer side of the rail, the metal "fishbelly" edge rail with its side rack being cast all in one piece, in lengths. Blenkinsop's system remained in use for 25 years on the Middleton Railway, but it became a curiosity because simple friction was found to be sufficient for railroads operating on level ground.

Two other searches for items at the Antikythera wreck site in 2012 and 2015 have yielded a number of fascinating art objects and a second ship which may or may not be connected with the treasure ship on which the Mechanism was found.. Also found was a bronze disc, embellished with the image of a bull. The disc has four "ears" which have holes in them, and it was thought by some that it may have been part of the Antikythera Mechanism itself, as a "cog wheel". However, there appears to be little evidence that it was part of the Mechanism; it is more likely that the disc was a bronze decoration on a piece of furniture..

Reflecting the rapid advances in propeller engine technology, the original engines were removed and replaced with a pair of smaller, lighter and more modern oscillating engines, with cylinders and stroke, built by John Penn & Sons of Greenwich. They were also provided with more support at the base, and supported further by the addition of both iron and wood beams running transversely across the hull, which had the added benefit of reducing engine vibration. The cumbersome chain-drive gearing was replaced with a simpler and by now proven cog-wheel arrangement, although the gearing of the engines to the propeller shaft remained at a ratio of one to three. The Aigle–Leysin railway line is long and rises from its terminus outside the main line station in Aigle to its summit at the Grand Hôtel at Leysin. The first 1 km of the route is through the streets of Aigle from the railway station to the railway depot where the train reverses to enable the powered vehicle to be at the rear of the train for the uphill journey, normal working on a rack (cog-wheel) railway. From this point the line climbs steeply through the vineyards, the steepest gradient being 1 in 4.3 (23%). The operating voltage of the line has changed four times, increasing from 600 V DC at opening, first to 650 V DC, then in 1946 to 1300 V DC, and later to the present operating voltage of 1500 V DC.

On steep grades the friction between the wheels and the rails cannot apply sufficient adhesion to the train's wheels so as to overcome gravity, and the train is in danger of sliding down the track. In practice this affects downhill braking capability before it affects uphill climbing ability, and some mountain railways at the lower end of the steepness spectrum rely on standard adhesion for propulsion, but use special track brakes acting directly on the rails. Where the line is too steep to rely on adhesion for climbing, a rack railway may be used, in which a toothed cog wheel engages with a toothed rack rail laid between the tracks. A now little used alternative to the rack and pinion railway is the Fell system, in which traction and/or braking wheel are applied to a central rail under pressure.
