Safar conducted research on existing basic life support procedures including controlling a person's airway by tilting back his or her head, opening their mouth by moving their chin downwards, and breathing air into the trachea. Beck first used the technique successfully on a 14-year-old boy who was being operated on for a congenital chest defect. The boy's chest was surgically opened, and manual cardiac massage was undertaken for 45 minutes until the arrival of the defibrillator. Beck used internal paddles on either side of the heart, along with procainamide, an antiarrhythmic drug, and achieved return of a perfusing cardiac rhythm. These early defibrillators used the alternating current from a power socket, transformed from the 110–240 volts available in the line, up to between 300 and 1000 volts, to the exposed heart by way of "paddle" type electrodes.

William Bennet Kouwenhoven (13 January 1886 – 10 November 1975), also known as the "Father of Cardiopulmonary Resuscitation," is famous for his development of the closed-chest cardiac massage and his invention of the cardiac defibrillator. After obtaining his doctorate degree in engineering from the Karlsruhe Technische Hochschule in Germany, Kouwenhoven began his career as the dean at the Johns Hopkins University in Baltimore. Kouwenhoven focused his research mainly on improving and saving lives of patients through the application of electricity. With the help and cooperation of the Johns Hopkins School of Medicine's Department of Surgery and an Edison Electric Institution grant, Kouwenhoven was able to develop a closed-chest defibrillator.

On 24 October 2010, during a home game against Real Betis (0–3 home loss), he suffered a cardiac arrest in the 57th minute and collapsed. He was attended by doctors of both teams, who revived him with cardiac massage and the use of a defibrillator; he left the stadium in a stable condition, subsequently undergoing tests at the Salamanca University Hospital. Glenn Russell Middlemiss (born 8 June 1956) is a former Australian rules footballer who played with Geelong and St Kilda in the Victorian Football League (VFL). He is the son of Russell Middlemiss, a Geelong premiership player in 1951 and 1952. Middlemiss played his early football at Geelong West in the Victorian Football Association.The Age, "Glenn takes his chances", 5 April 1977, p. 17 While playing for Geelong West, against Williamstown in 1977, Middlemiss was knocked unconscious in an accidental collision, after which he was clinically dead for two-and-a-half minutes. The match was stopped for 20 minutes as he was attended to by trainers and a doctor, who gave him mouth-to-mouth resuscitation, cardiac massage and oxygen.

Cemil Topuzlu during his military service On 27 August 1903, one of his patients undergoing external urethrotomy under chloroform anaesthesia developed cardiac arrest, and he performed open chest cardiac massage. He also defined the "Do not resuscitate" code in cases involving serious heart disease and other diseases, where life expectancy is very short. He introduced novel vascular suture techniques, which he presented at the International Medical Congress in Moscow in August 1897 and at the annual Congress of the Société de Chirurgie de Paris in July 1904, where he reported two cases of arterial tear during breast carcinoma resection and repair within the same session. He also reported the removal of a pen cover from the right main bronchus of a 7-year- old girl through a tracheotomy in 1903.

Either a classical midine incision or a Pfannenstiel incision may be used depending on operator preference; the former may theoretically give better exposure, but practising obstetricians or surgeons may be more comfortable with a Pfannenstiel approach as this is more commonly used for Caesarean sections. Once the uterus is opened, the fetus is delivered and should be resuscitated by a separate team. It may be possible to then use the abdominal incision to deliver direct cardiac massage through the (intact) diaphragm. After the placenta is delivered, the uterus is massaged to stimulate contraction and is closed with a running locking absorbable suture and the abdomen is then closed; alternatively, the wound may be temporarily packed with sterile gauze, with definitive closure delayed until specialist obstetric help arrives or until the patient is fit for transport to a formal operating theatre.
