The refusal of medical assistance, or RMA, ensures the continuum of care that ambulance squads have a responsibility towards. In a typical emergency call, the ambulance service will assess and transport the patient to an appropriate facility. The ambulance squad's duty towards the patient begins with patient contact and generally ends with transfer to the emergency department of the receiving hospital. The call can terminate in other ways; for example an ambulance service may cancel their own services if the patient becomes violent (scene safety), if they are canceled by on-scene personnel (such as the police or, in the case of a MICU service, the on-scene BLS EMT), or at the discretion of the dispatcher (if another ambulance is better positioned to take the call).

The film received a mainly negative reception from critics and was criticized for its violence; Vincent Canby of the New York Times described it as "violent, but spiritless." Variety wrote "Lundgren can hold his own with other action leads as an actor and could easily be Van Damme-marketable if only he'd devote as much attention to quality control as he does to pectoral development." David J. Fox of the Los Angeles Times, however, described the film as a "class act", and some retrospective critics find it to be entertaining for its genre.

The secret of his blackness is one that he abhors as well as cherishes; he often willingly tells white people that he is black in order to see their extreme reactions and becomes violent when one white Northern woman reacts nonchalantly. Though Christmas is guilty of violent crimes, Faulkner emphasizes that he is under the sway of social and psychological forces that are beyond his control and force him to reenact the part of the mythical black murderer and rapist from Southern history. Christmas exemplifies how existing outside of categorization, being neither black nor white, is perceived as a threat by society that can only be reconciled with violence. He is also perceived as neither male nor female, just as Joanna Burden, whom Faulkner portrays as "masculinized," is also neither male nor female and is rejected by her community.
