An underlying malignancy is found in 50% of cases. Usually, it occurs in those afflicted by a life-threatening illness. This phenomenon was discovered by Jonathan Towne, a vascular surgeon in Milwaukee, who was also the first to report the "white clot syndrome" (now called heparin induced thrombocytopenia [HIT]). If tissue is not being perfused properly, it may feel cold and appear pale; if severe, hypoxia can result in cyanosis, a blue discoloration of the skin. If hypoxia is very severe, a tissue may eventually become gangrenous. Extreme pain may also be felt at or around the site. Tissue hypoxia from low oxygen delivery may be due to low haemoglobin concentration (anaemic hypoxia), low cardiac output (stagnant hypoxia) or low haemoglobin saturation (hypoxic hypoxia).

Cryoglobulinemia is a medical condition in which the blood contains large amounts of pathological cold sensitive antibodies called cryoglobulins – proteins (mostly immunoglobulins themselves) that become insoluble at reduced temperatures. This should be contrasted with cold agglutinins, which cause agglutination of red blood cells. Cryoglobulins typically precipitate (clumps together) at temperatures below normal body temperatureand will dissolve again if the blood is heated. The precipitated clump can block blood vessels and cause toes and fingers to become gangrenous.

He was shot down on 5 September after getting two Albatros D.VIIs himself. His last flight was just before he was to go on home leave by the end of which the war would be over. He was taken prisoner and his right leg, which had become gangrenous from a bullet wound in his ankle, was amputated at the upper thigh. He was repatriated on 20 December 1918 to the Prince of Wales Hospital for Officers, Marylebone.

The damaged tissue may become gangrenous and eventually slough away. Initially there may be no pain from a bite, but over time the wound may grow to 10 inches (25 cm) in extreme cases. Bites usually become painful and itchy within two to eight hours, pain and other local effects worsen 12 to 36 hours after the bite, and then necrosis will develop over the next few days. Systemic effects are unusual but include mild nausea, vomiting, fever, rashes, and muscle and joint pain.

Purpura fulminans lesions, once established, often progress within 24 to 48 hours to full-thickness skin necrosis or soft-tissue necrosis. Once purpura fulminans lesions progress to full-thickness skin necrosis, healing takes between 4–8 weeks and leaves large scars. Without treatment, necrotic soft tissue may become gangrenous, leading to loss of limbs. Purpura fulminans is often accompanied by micro-vascular thrombosis and haemorrhagic infarction in other tissues, such as the lungs, kidneys, central nervous system and adrenal glands, leading to multiple organ failure, and causes initial high mortality and long-term morbidity in survivors.
