Unlike medicine, an alternative product or practice does not originate from using scientific methods, but may instead be based on hearsay, religion, tradition, superstition, belief in supernatural energies, pseudoscience, errors in reasoning, propaganda, fraud, or other unscientific sources.

An article in the British Medical Journal about European tortures describes a method of tickle torture in which a goat was compelled to lick the victim's feet because they had been dipped in salt water. Once the goat had licked the salt off, the victim's feet would be dipped in the salt water again and the process would repeat itself. However, it remains unclear if this method was ever used in practice as it is only described in the 1502 Tractatus de indiciis et tortura by the Italian jurist and monk Franciscus Brunus de San Severino - a treatise that actually cautioned against torture in general - and while it seems clear that Franciscus Brunus had not made up this practice, the issue is left open whether the inclusion in the treatise is based on hearsay, (reliable) eye- witness accounts or personal experience.

The incidence of genetic monogamy may be estimated from rates of extrapair paternity. Extrapair paternity is when offspring raised by a monogamous pair come from the female mating with another male. Rates of extrapair paternity have not been extensively studied in people. Many reports of extrapair paternity are little more than quotes based on hearsay, anecdotes, and unpublished findings. Simmons, Firman, Rhodes, and Peters reviewed 11 published studies of extra-pair paternity from various locations in the United States, France, Switzerland, the United Kingdom, Mexico, and among the native Yanomami Indians of Amazon forest in South America. The rates of extrapair paternity ranged from 0.03% to 11.8% although most of the locations had low percentages of extrapair paternity. The median rate of extrapair paternity was 1.8%. A separate review of 17 studies by Bellis, Hughes, Hughes, and Ashton found slightly higher rates of extrapair paternity. The rates varied from 0.8% to 30% in these studies, with a median rate of 3.7% extrapair paternity.

In an article published in the British Medical Journal in 1979, Dr F. Kräupl Taylor reviewed the literature on penis captivus and concluded that while "almost all the cases mentioned in medical publications and in textbooks are based on hearsay and rumour", two papers published by nineteenth-century German gynaecologists – Scanzoni (1870) and Hildebrandt (1872) – who had personally dealt with cases of the condition "leave no doubt about the reality of this unusual symptom", which, however, "is so rare that it is often regarded nowadays as no more than a prurient myth". There was considerable controversy solely based on hearsay over whether or not using tapes from different manufacturers could lead to dropouts.
