The medical history, case history, or anamnesis (from Greek: ἀνά, aná, ″open″, and μνήσις, mnesis, ″memory″) of a patient is information gained by a physician by asking specific questions, either of the patient or of other people who know the person and can give suitable information, with the aim of obtaining information useful in formulating a diagnosis and providing medical care to the patient. The medically relevant complaints reported by the patient or others familiar with the patient are referred to as symptoms, in contrast with clinical signs, which are ascertained by direct examination on the part of medical personnel. Most health encounters will result in some form of history being taken. Medical histories vary in their depth and focus.

The earliest and most vivid account of Carpocrates and his followers comes from Irenaeus (died 202) in his Against HeresiesIrenaeus: Against Heresies (Excerpt) including an account of the theology and practice of the sect. They believe, he writes, that Jesus was not divine; but because his soul was "steadfast and pure", he "remembered those things which he had witnessed within the sphere of the unbegotten God" (similar to Plato's concept of Anamnesis). Because of this, Jesus was able to free himself from the material powers (what other Gnostics call Archons, the Demiurge, etc.). Carpocratians believed they themselves could transcend the material realm, and therefore were no longer bound by Mosaic law, which was based on the material powers, or by any other morality, which, they held, was mere human opinion.

There are some Judaic mystical traditions that explain how and why illuim are different. One explanation attributes it to an "Angel's Slap": :An intriguing legend from the Talmud and Midrash describes how a child, while still in its mother's womb, is taught the entire Torah to the glow of a supernatural lamp that allows it to see to the ends of the earth. It is only at the moment of birth that an angel appears and imposes upon it an oath to live a righteous life, and then slaps the youngster on the mouth or the nose, causing it to forget all that it has learned. :The angel's smack in the Talmudic legend produces total amnesia for all, but in the Greek theory of "anamnesis" the souls quaff varying quantities of the oblivion-inducing potion.

Seabury played a decisive role in the evolution of Anglican liturgy in North America after the Revolution. His "Communion Office," published in New London in 1786, was based on the Scottish Liturgy of 1764 rather than the 1662 Book of Common Prayer in use in the Church of England. Seabury's defense of the Scottish service-- especially its restoration of oblationary language and the epiklesis or invocation of the Holy Spirit in the Prayer of Consecration was adopted into the Book of Common Prayer with minor change by the Episcopal Church in 1789. The English 1552, 1559, 1604 and 1662 Books of Common Prayers of Consecration ended with the Words of Institution; but the Scottish Rite Prayer continued with an oblation, anamnesis, epiclesis, intercessions and doxology based on the ancient classical models of consecration prayers.

Stage play adaptations under the title Persona 5: The Stage have been performed in Japan, with original music composed by Atsushi Kitajoh. Characters and other elements from the game have also made appearances in Dragon's Dogma Online, Phantasy Star Online 2, Lord of Vermilion Re:3, Sonic Forces, Puzzle & Dragons, Granblue Fantasy, Catherine: Full Body, Star Ocean: Anamnesis, Another Eden, and Tokyo Mirage Sessions ♯FE Encore. Joker also appears as a playable character via downloadable content (DLC) in the 2018 crossover fighting game, Super Smash Bros. Ultimate. The game's director, Masahiro Sakurai, is a fan of the Persona series and stated that Joker was emblematic of the approach that he wanted to take with Ultimate DLC, adding that he wanted characters that were "unique", "different", and "fun" to use within the Super Smash Bros. environment.

The followers of John Wesley have typically affirmed that the sacrament of Holy Communion is an instrumental Means of Grace through which the real presence of Christ is communicated to the believer, but have otherwise allowed the details to remain a mystery. In particular, Methodists reject the Roman Catholic doctrine of transubstantiation (see "Article XVIII" of the Articles of Religion); the Primitive Methodist Church, in its Discipline also rejects the Lollardist doctrine of consubstantiation. In 2004, the United Methodist Church affirmed its view of the sacrament and its belief in the real presence in an official document entitled This Holy Mystery: A United Methodist Understanding of Holy Communion. Of particular note here is the church's unequivocal recognition of the anamnesis as more than just a memorial but, rather, a re-presentation of Christ Jesus and His Love.

Text and melody are based on a Leise, a German congregational refrain ending on Kyrie eleison, of the Latin sequence Lauda Sion for Corpus Christi. Luther knew it in a version which first appeared at the end of the 14th century in a processional from the Franciscan monastery Miltenberg, made in Mainz, at the end of the 14th century: Luther praised the Leise in his writing Von der Winkelmesse und Pfaffenweihe in 1533, appreciating that it is focused on the sacrament of bread and wine, not on sacrifice. He made it a song of thanks after communion, by shortening the text for a first stanza, and by adding two stanzas. The second stanza mentions the anamnesis of the gifts of redemption, the third stanza is a prayer for spiritual fruits of the sacrament for the individual life of the Christian, and for the community.

Within the Torah, the showbread is mentioned exclusively by the Priestly Code and Holiness Code, but certain sections of the Bible, including the Book of Chronicles, Books of Samuel, and Book of Kings, also describe aspects of them. In the Holiness Code, the showbread is described as twelve cakes/loaves baked from fine flour, arranged in two rows/piles on a table standing before God; each loaf/cake was to contain two omers of flour () (approx 4.9 pounds). The Biblical regulations specify that cups of frankincense were to be placed upon the rows of cakes, and the Septuagint, but not the masoretic text, states that salt was mixed with the frankincense; the frankincense, which the Septuagint refers to as an anamnesis, (a hapax legomenon), constituted a memorial (azkarah), having been offered upon the altar to God ().

The anamnesis or medical history taking of issues related to reproductive or sexual medicine may be inhibited by a person's reluctance to disclose intimate or uncomfortable information. Even if such an issue is on the person's mind, he or she often doesn't start talking about such an issue without the physician initiating the subject by a specific question about sexual or reproductive health. Some familiarity with the doctor generally makes it easier for person to talk about intimate issues such as sexual subjects, but for some people, a very high degree of familiarity may make the person reluctant to reveal such intimate issues. When visiting a health provider about sexual issues, having both partners of a couple present is often necessary, and is typically a good thing, but may also prevent the disclosure of certain subjects, and, according to one report, increases the stress level.

Robert Shearman and Lars Pearson, in their book Wanting to Believe: A Critical Guide to The X-Files, Millennium & The Lone Gunmen, rated several episodes across the season highly, awarding five stars out of five to "The Curse of Frank Black", "Jose Chung's Doomsday Defense", "Midnight of the Century", "A Room with No View", "The Fourth Horseman" and "The Time Is Now". However, several episodes rated poorly, with "Beware of the Dog", "A Single Blade of Grass", "The Hand of St. Sebastian", "Roosters", "In Arcadia Ego" and "Anamnesis" being seen as particularly poor. In all, Shearman singled out the season's climactic two-part episodes "The Fourth Horseman" and "The Time Is Now" as the highlight of the season, finding that Morgan and Wong took a considerable risk with the storyline as they believed the series would be cancelled at the end of the season, but believing that the episodes gained from this additional level of attempted closure.

Anglicans adhere to a range of views although the teaching in the Articles of Religion holds that the body of Christ is received by the faithful only in a heavenly and spiritual manner. Some Christians do not believe in the concept of the real presence, believing that the Eucharist is only a ceremonial remembrance or memorial of the death of Christ. The Baptism, Eucharist and Ministry document of the World Council of Churches, attempting to present the common understanding of the Eucharist on the part of the generality of Christians, describes it as "essentially the sacrament of the gift which God makes to us in Christ through the power of the Holy Spirit", "Thanksgiving to the Father", "Anamnesis or Memorial of Christ", "the sacrament of the unique sacrifice of Christ, who ever lives to make intercession for us", "the sacrament of the body and blood of Christ, the sacrament of his real presence", "Invocation of the Spirit", "Communion of the Faithful", and "Meal of the Kingdom".

The success of the play at the Petit Odéon and the led him, after several failures with other creations, to resume the process of dialogue between two characters, each incarnating a cause opposite to the other, Descartes and Blaise Pascal for reason and faith, the marquise du Deffand and Julie de Lespinasse for the old and the modern, Talleyrand and Fouché for political genius and arrivism, Napoléon and Hudson Lowe for tragic destiny and honest pettiness. It is in L'Antichambre that he expressed all his melancholy for a French language in the process of disappearance. In 1984, he approached René Char, a brother in writing of Albert Camus. Starting in 1997, he began a work of "anamnesis" "Not to be alien to oneself" which he published, faithful to existentialism, in the form of fragments of the past which related less to his person than to their times, memories mixed with aphorisms with pessimistic humor.

The preparation for the lessons (the little Entrance) and the carrying of the oblation from the Prothesis to the altar (the Great Entrance) become solemn processions, but the outline of the liturgy: the Mass of the Catechumens and their dismissal; the litany; the Anaphora beginning with the words "Right and just" and interrupted by the Sanctus; the words of Institution; Anamnesis, Epiklesis and Supplication for all kinds of people at that place; the Elevation with the words "Holy things to the holy"; the Communion distributed by the bishop and deacon (the deacon having the chalice); and then the final prayer and dismissal–this order is characteristic of all the Syriac and Palestinian uses, and is followed in the derived Byzantine liturgies. Two points in that of the Apostolic Constitutions should be noticed. No saints are mentioned by name and there is no Our Father. The mention of saints' names, especially of the "All-holy Mother of God", spread considerably among Catholics after the Council of Ephesus (431), and prayers invoking her under that title were then added to all the Catholic liturgies.

Music criticism in France has pointed out that Juan Manuel Abras is one of the European composers who "represent a good portion of the Europe of the new music" and, at the same time, music criticism in Buenos Aires has considered him "a genuine representative of Argentine music". In Germany, Genuin classics has highlighted "the breathless intensity of Juan Manuel Abras" and within the French musicological field it has been stated that "variety is also found in his musical language", a language that contains "a unity, by nature inaudible, which is realised each time through the different styles",Ibid. maintaining that in his works "there are, apart from the psychoanalysis [...] references to literature, religion, archaeology, etc.".Ibid. The Spanish academic field has gathered that Abras' music is "cosmopolitan and multifaceted, in search of a balance between emotion and reason, a dialogue between past and future, tending towards the Absolute" and that "it contains influences from the composers with whom he studied, such as Penderecki, Stockhausen, Lachenmann and Rihm, equally nourishing from concepts such as anamnesis, union of the opposites, numinosity, ekphrasis and intertextuality".Ibid.

Often when compared with the Latin Church the meaning of Anaphora and Liturgy can be mixed up. However, there is a clear distinction in the Syriac Church. The Liturgy of St James the Just is the skeleton of the whole Qurbono Qadisho with all the prayers before the Anaphora being exactly the same no-matter which anaphora used.
