Typically, pilgrims arrive at the Accueil Notre Dame in specially adapted buses, either from Lourdes airport or train station, and will be welcomed in the transit lounge from where they are taken to their rooms. Another accueil, the Accueil Marie St. Frai, is located a short distance outside the domain; it is similar in design and atmosphere to the Accueil Notre Dame.

The other section is now known as the Accueil John Paul II, and contains several chapels (e.g. St Cosmos & St Damien), the First Aid post and Dispensary, and the offices of the Hospitalité. The other was the Accueil St. Bernadette, which stood across the river from the old Accueil Notre Dame, and was demolished to make way for the new one. Since Easter 1997 sick pilgrims from all over the world have been housed in the Accueil Notre Dame, an airy modern building.

The Accueil Notre Dame Across the river from the grotto and the churches is the Accueil Notre Dame, a modern facility built in 1996 to house sick pilgrims during their time in Lourdes. The Chapel of Reconciliation formerly occupied a site slightly more remote, at the entrance of the Upper Stations of the Cross. It was moved several years ago into a more prominent position, into the building previously known as the Accueil Notre Dame, near the Crowned Statue and facing the Esplanade. The Chapel of Reconciliation is somewhat unusual in that no Masses or other services take place there; instead it is given over entirely to the Sacrament of Reconciliation. Priests from different countries observe a duty roster, which means that, at almost any time of day, pilgrims from Europe (and occasionally further away) can find a priest who will hear their confession in their own language.

The Pilgrimage travel is organised by the current Director, Fr David Long, and a team of unpaid volunteers. Although some pilgrims make their own way, the vast majority travel on the Diocesan coaches and ferry to the port of Calais in the North of France, and then on to Lourdes on chartered TGVs. The team also arranges the charter of a flight for those Pilgrims who are unable to travel for the 24 hours overland. Pilgrims can stay in a range of hotels, available for most budgets, whilst those who need constant care or medical support are accommodated in the Accueil Notre Dame where they are looked after by a medical team, led by the Matron, of doctors and nurses.

The Chief Brancardier is assisted in the day-to-day running of the duties by a team of deputies, and by the Duty Team Leaders. The medical care of pilgrims is undertaken by a team of doctors and nurses, overseen by the Pilgrimage Matron, who also meet throughout the year to plan for the pilgrimage. In a similar manner to the teams of helpers, the nurses work on a shift rota, with different teams taking different duties, and it is the senior nurse on duty who is responsible for supervising the provision of care on the wards in the Accueil. Currently the Hospitalité are responsible for co- ordinating almost 600 helpers, including almost 500 youth, and assisting approximately 200 pilgrims who require daily assistance during their stay in Lourdes.

Charles VI (or Charles VII) leaving the Hôtel Saint-Pol, miniature in the Chronique (15th century) of (Bibliothèque Nationale de France)According to Lorentz and Sandron 2006, pp. 91, 99, this miniature depicts the Hôtel Saint- Pol and the funeral of Charles VI, c. 1422, but according to the BnF, Département des Manuscrits, Accueil, it depicts the funeral of Charles VII of France. The Hôtel Saint-Pol was a royal residence begun in 1360 by Charles V of France on the ruins of a building constructed by Louis IX. It was used by Charles V and Charles VI. Located on the Right Bank, to the northwest of the Quartier de l'Arsenal in the 4th arrondissement of Paris, the residence's grounds stretched from the Quai des Célestins to the Rue Saint-Antoine, and from the Rue Saint-Paul to the Rue du Petit-Musc.
