The Battle of Loup Hill took place near Loup Hill () in Kintyre on 16 May 1689, during the Jacobite rising of 1689, a connected conflict of the Williamite War in Ireland. In early May, Jacobite sympathisers took control of northern Kintyre, and when government troops arrived to retake the peninsula, they ran into an ambush on the slopes of Loup Hill. Despite the advantage of height and surprise, the Jacobites fought ineffectively and quickly retreated; a few days later, they abandoned Kintyre. Losses on both sides were minimal and the battle itself was little more than a minor skirmish.

Until 1975, the Royal Lao Army were the armed forces of the Kingdom of Laos. Serving one of the world's least developed countries, the Lao People's Armed Forces (LPAF) is small, poorly funded, and ineffectively resourced. Its mission focus is border and internal security, primarily in internal suppression of Laotian dissident and opposition groups. This includes the brutal crushing of the peaceful 1999 Lao Students Movement of Democracy demonstrations in Vientiane, and in countering ethnic Hmong insurgent groups and other groups of Laotian and Hmong people opposing the one-party Marxist-Leninist Pathet Lao government and the support it receives from the Socialist Republic of Vietnam.

Al-Bazi was vicar of the St. Elia (alt: Elias) Catholic Church, and the adjacent St. Elia Catholic School in the "working class" New Baghdad neighborhood of Baghdad. By 2010, the school had an 82 percent Muslim enrollment, as a result of the exodus of Christians from Iraq. In his autobiography, Norman Kember relates that Al-Bazi described his church as having been bombed ineffectively by unidentified anti-Christian elements and as a church where members of several denominations worshiped together. At the time of Valenzuela's release, several Dodgers leaders, including Tommy Lasorda, Fred Claire, and Peter O'Malley, praised Valenzuela for creating exciting memories over several seasons and they indicated that it was a difficult decision to release him. After his death, confidential papers were said to have been removed from his house by British agents and his heirs were paid not to republish the book - ineffectively, since it was republished as A Political and Military Rhapsody on the Invasion and Defence of Great Britain in 1794 and 1798 when French invasion was again feared. Shortly after the battle, Adherbal was reinforced by Carthalo with 70 ships. Adherbal brought Carthalo's command up to 100 and sent him to raid Lilybaeum, where he burnt several Roman ships. A little later, he harried a Roman supply convoy of 800 transports, escorted by 120 warships, to such good effect that it was caught by a storm which sank all the vessels except for two. The so-called "rural periphery" or the land that was located outside a two-hour radius from an urban center was being cultivated inefficiently and ineffectively. The way in which the land was cultivated was done so unwisely—too much of the land was farmed at one time in contrast to using a structure like that of crop rotation, which uses modern irrigation techniques and a variance of crops. In other words, what was going on can be simplified as "the greater the amount of land under cultivation, the lower the productivity", which led ultimately led to a lack in fertility. Landowners began to abandon their land and failed to report the actual size of their property to the Federal Statistical Agency.

The greatest concern facing lemur populations is habitat destruction and degradation. Deforestation takes the form of local subsistence use, such as slash and burn agriculture (referred to as tavy in Malagasy), the creation of pasture for cattle through burning, and legal and illegal gathering of wood for firewood or charcoal production; commercial mining; and the illegal logging of precious hardwoods for foreign markets. After centuries of unsustainable use, as well as rapidly escalating forest destruction since 1950, less than 60,000 km2 (23,000 sq mi) or 10% of Madagascar's land area remains forested. Only 17,000 km2 (6,600 sq mi) or 3% of the island's land area is protected and due to dire economic conditions and political instability, most of the protected areas are ineffectively managed and defended.

Moore's last issue, number 16 ("Olympus") ends with a depiction of Miracleman's apotheosis, as he and his superhuman allies bring the entire planet under their totalitarian control. Miracleman and his companions, explicitly compared to gods, now rule the planet as they see fit, though they are ineffectively opposed by groups such as an alliance of Christian and Islamic fundamentalists.

As Nazi Germany faced severe defeat after the Battle of Kursk and especially the cross-channel invasion it introduced cross- channel use of the V-1 flying bomb and V-2 rocket, although too late and too ineffectively to turn the war to its advantage. The German military machine was developing jet aircraft as fighters and bombers and long-range missiles, but far too late (they were only in the design and test stages) to change the outcome of the war. The victorious Allies would incorporate the early innovations of jet technology and long-distance rocket-based missiles into their armed forces, but only after the end of World War II after getting them beyond the developmental stages of design and testing.

Cicotte in 1913 The book Eight Men Out by Eliot Asinof and the movie based on the book does record that Cicotte, despite being grossly underpaid for a pitcher of his ability, resisted repeated attempts by Chick Gandil to get him to throw the series until just days before the World Series opened when it became clear that Comiskey would never pay him even part of the promised bonus. In the 1919 World Series against the Reds, Cicotte pitched in three games, winning one but pitching ineffectively and losing the other two. Cicotte was the first of the eight players to come forward, signing a confession and a waiver of immunity. He later recanted this confession and was acquitted of all charges at trial by jury.

During early 1944, the B-29 heavy bombers were operating ineffectively from bases in China. Stationing the Superfortresses in the Marianas brought Japan within their effective range of operation, as well as provided the Twentieth Air Force with reliable means of support from the western ports of the United States. The Ushi Point Airfield and its assigned aircrews did their part to repel American advances in the Marianas Islands but following the fall of Guam and Saipan to American forces in July 1944 it became clear that Tinian would be attacked next. Assaulted on July 24, 1944 by United States Marines from Saipan, which had just been taken the previous month, the airfield was almost totally destroyed by the American naval bombardment and air attack prior to the assault by the 4th Marine Division.

In Great Britain, questions were raised in Parliament implying that the screening in April had been improperly or ineffectively carried out. Japanese press opinion reflected a growing suspicion that the US authorities had lost control of the screening process and permitted South Korean Government pressure to be exerted directly or indirectly against repatriation. As General Jenkins, Army G-3, pointed out to General Collins early in June: "The cumulative effect of sentiment such as that reflected above may tend to obscure the UNC principle of no forcible repatriation, and appear to make the armistice hinge on the questionable results of a discredited screening operation." The article also reported that women as far along as 8.5 months pregnant were forced to abort, usually by an injection of saline solution. A 1993 book by social scientist and anti-abortion political activist Steven W. Mosher reported that women in their ninth month of pregnancy, or already in labour, were having their children killed whilst in the birth canal or immediately after birth. According to a 2005 news report by Australian Broadcasting Corporation correspondent John Taylor, China outlawed the use of physical force to make a woman submit to an abortion or sterilization in 2002 but ineffectively enforces the measure. In 2012, Feng Jianmei, a villager from Shaanxi province was forced into an abortion by local officials after her family refused to pay the fine for having a second child.

The United States Court of Appeals for the Fifth Circuit, in a three judge panel, determined that Washington had been ineffectively represented. However, the state of Florida appealed the decision, and the appeal occurred while the United States Court of Appeals for the Eleventh Circuit was statutorily created from a part of the Fifth Circuit. The United States Court of Appeals for the Eleventh Circuit en banc, reversed the Fifth Circuit's ruling, stating that the Sixth Amendment accorded [466 U.S. 668, 669] criminal defendants a right to counsel rendering "reasonably effective assistance given the totality of the circumstances." After outlining standards for judging whether a defense counsel fulfilled the duty to investigate nonstatutory mitigating circumstances and whether counsel's errors were sufficiently prejudicial to justify reversal, it remanded the case for application of the standards.

Over two years prior, on 30 September 2011, the Office of the Prime Minister (OPM) transferred Thawin Pliansri from the office of Secretary General of the National Security Council (NSC) to the office of Prime Minister Advisor. Police General Wichian Photphosri, then Commissioner General of the Royal Thai Police (RTP), was appointed as the NSC Secretary General instead, and Police General Priewpan Damapong, elder brother of Thaksin's former wife Potjaman Shinawatra, was appointed as the new RTP Commissioner General. Thawin requested an administrative court to revoke the transfer. The Supreme Administrative Court delivered a final judgment on 7 March 2014 that the transfer was legal, but was unreasonable because it did not appear that Thawin had carried out his duties ineffectively or failed to abide by the policy of the government.

Soviet PT-76s along with T-54s, T-55s, and Chinese Type 59s, Type 62 tanks formed the bulk of the People's Army of Vietnam armored forces. The first successful action of NVA armor in Vietnam was against the Lang Vei Special Forces camp on 6/7 February 1968Dunstan (they had already been used in the preceding Battle of Ban Houei Sane, which was just across the border in Laos however). Thirteen PT-76s, of the NVA 202nd Armored Regiment spearheaded an assault against approximately 24 Green Berets, 500 Vietnamese irregulars and 350 Laotian Royal soldiers. The defenders fought back with their 106 mm M40 recoilless rifle (one at the entrance took out three PT-76s until it was knocked out), and ineffectively with M72 LAWs (one-shot disposable 66 mm Light Anti-Tank Weapon).

While serving in the Bahamas, Blankenship became best known for his emphasis on drug interdiction. Notable in his efforts was the added use of electronic intelligence gathering and small guerrilla actions against the cartels, which the US Coast Guard in joint efforts with the DEA had been practicing for some time ineffectively.

JCIDS was developed under the direction of Secretary of Defense Donald Rumsfeld to address shortfalls in the United States Department of Defense (DoD) requirements generation system identified by the U.S. Joint Chiefs of Staff. These shortfalls were identified as: not considering new programs in the context of other programs, insufficiently considering combined service requirements and ineffectively prioritizing joint service requirements, and accomplishing insufficient analysis. The drive to create JCIDS was born out of a March 2002 Secretary of Defense memorandum to the Vice Chairman of the Joint Chiefs of Staff requesting a study on alternative ways to evaluate requirements. The Chairman of the Joint Chiefs of Staff (CJCS) approved the most recent JCIDS Instruction on 23 January 2015"Joint Capabilities Integration and Development System (JCIDS)", CJCSI 3170.01I 23 January 2015 and its accompanying manual was released on 12 February 2015.

Whether serving in an academic medical center, working as a physician executive in a health plan or public health program, functioning as a partner in a group practice, collaborating on a community project, or advocating for effective health legislation, physicians have multiple opportunities to function as leaders in changing health care in the 21st century. Despite these prospects, many physicians either have not taken on leadership roles or function ineffectively in those roles.

Even in those jurisdictions without formal "actual innocence" provisions in their post-conviction statutes, actual innocence can have a procedural effect, in that it will excuse procedural default and permit the filing of a successor collateral relief petition. This is based on the U.S. Supreme Court's decision in Schlup v. Delo, 513 U.S. 298 (1995), in which a death row inmate filed a second federal habeas corpus petition, asserting as substantive claims the claims that his trial lawyer had ineffectively failed to present alibi witnesses and that the Government had wrongly concealed exculpatory evidence. Schlup also argued that he was actually innocent—not because that was a substantive ground for relief, but because his actual innocence excused his failure to raise his ineffective- counsel and prosecutorial-nondisclosure claims in his state court pleadings and in his first federal habeas petition.

The tax force recommended the adoption of a $75 million head tax, at a rate of up to $480 per employee, to fund homelessness services. The proposal was once again criticized by a consortium of business leaders as a tax on jobs or one targeted at Amazon, also accusing the city government of ineffectively using taxpayer revenue allocated for homelessness services. The full proposal was released in April 2018, outlining a tax on large employers with annual revenues of over $20 million, of which roughly 500 businesses would be affected. The tax would generate $75 million in annual revenue with a per- employee tax of $540, but would be replaced by a 0.7 percent payroll tax in 2021. A study from the city council stated that 75 percent of the revenue would be used to build 2,000 units of affordable housing, while 20 percent would be used on homeless services.

In one of those games, on July 25, Warhop shutout the Chicago White Sox for twelve innings before losing 1–0 in the thirteenth, due to two errors—one of which was committed by himself on an errant throw to first base after a sacrifice bunt. His first win of the season finally came on June 25 against the Boston Red Sox.

This was not the first or the last example of controversial remarks by McCain. A week later on March 7, 2000, he lost nine of the thirteen primaries on Super Tuesday to Bush, including large states such as California, New York, Ohio, and Georgia; McCain's wins were confined to the New England states of Massachusetts, Rhode Island, Connecticut and Vermont. His overall loss on that day has been attributed to his going "off message", ineffectively accusing Bush of being anti-Catholic due to having visited Bob Jones University and getting into a verbal battle with leaders of the Religious Right. Throughout the campaign, McCain had achieved parity with Bush among self-identified Republicans only in the northeastern states; in most of the rest of the country, Bush ran way well ahead of McCain among Republicans, enough to overcome McCain's strength among independents and Democrats.

The murder of women in Juárez has attracted global attention since 1993 given suspected police and government inaction to prevent the murders and bring perpetrators to justice. Police and government officials have been accused of responding with indifference to the crimes against women as well as exhibiting tolerance for such crimes, conducting inadequate and negligent investigations, ineffectively responding to the crimes, and failing to prevent and protect women from violence. According to Livingston, "In 1998 the National Commission for Human Rights issued a report charging gross irregularities and general negligence in state investigations, including the misidentification of corpses, failure to obtain expert tests on forensic evidence, failure to conduct autopsies or obtain semen analysis... failure to file written reports, [and] incompetence in keeping records of the rising tide of women murders." As a result of international attention, police and government officials have been politically pressured to respond to the murders.

In United States law, ineffective assistance of counsel is a claim raised by a convicted criminal defendant asserting that the defendant's legal counsel performed so ineffectively that it deprived the defendant of the constitutional right guaranteed by the Assistance of Counsel Clause of the Sixth Amendment to the United States Constitution. Ineffectiveness claims may only be brought where the defendant had the right to counsel, ordinarily during the “critical stages” of a prosecution. Having the "benefit of counsel" or "assistance of counsel" means that the criminal defendant has had a competent attorney representing them. Competence is defined as reasonable professional assistance and is defined in part by prevailing professional norms and standards. To prove they received ineffective assistance, a criminal defendant must show two things: # Deficient performance by counsel # Resulting prejudice, in that but for the deficient performance, there is a “reasonable probability” that the result of the proceeding would have differed.

Destroyers also participated heavily on both sides of these battles and provided essentially all the torpedoes on the Allied side, with some battles in these campaigns fought entirely between destroyers.
